Big pain. Now I use the Bad Behavior plugin on most of my blogs and it basically hides your blog from bots. Cut the spam right down to almost nothing. On a few of my newer blogs I've been trying wp-spamfree and it's working equally as well (I think they can be used together too, but I haven't tried that, plus both work with Akismet activated). Both of these plugins are easy to install although you do have to do a little bit of a setup (easy) for bad behavior.
